I had the same problem with Patrick 100 and now an inherited 122. Once the pad has been flattened the straps rotate no matter what. The Starboard style insert help a bit but do not solve the problem. The problem with the Patrick was the pad material, that completely compressed to almost nothing. My solution was to cut of the pad around the insert, say 4cm x 4cm, and glue in a new section of good pad material. After that, and adding an anti-twist starboard-style washer, the strap stopped rotating.
The only alternative that works without replacing a section of the pad seems to be the S4LT anti-twist washers
